- Post-traumatic stress disorder (PTSD)
- Amyotrophic lateral sclerosis (ALS)
- Crohn’s disease
- Parkinson’s disease
- Multiple sclerosis (MS)
Florida also permits a terminal condition diagnosed by another physician, and chronic nonmalignant pain caused by—or originating from—a qualifying condition and continuing beyond its usual course.
What if my condition isn’t listed?
Florida also allows consideration of certain medical conditions comparable to those listed above. Your physician will evaluate your diagnosis and determine whether it meets the state’s requirements.
Your path to medical marijuana certification
1. Talk through your health goals
Your in-person visit at Elite Health DPC focuses on your symptoms, health history, current medications, and what you hope to achieve with treatment.
2. Receive an individualized assessment
Your physician assesses whether your diagnosis meets Florida’s requirements and whether the expected benefits of medical marijuana outweigh the potential risks.
3. Connect with Florida’s registry
If approved, your physician enters your certification and orders in the Medical Marijuana Use Registry. New registry users receive email instructions to access their account.
4. Complete your state card application
Submit the required documents and state fee through your registry account. Once your application is approved and your physician’s orders are active, you can purchase from a Florida-licensed dispensary.
Your state card renews annually. Physician reassessment is required at least every 30 weeks (210 days). State card guidance · Certification requirements
Questions before your appointment
Can I discuss anxiety, insomnia, or other health concerns during my evaluation?
Yes. At Elite Health DPC, your physician will review your symptoms, medical history, and previous treatments to determine whether you meet Florida’s eligibility requirements and whether medical marijuana may be appropriate for your care. Bring any relevant medical records and questions to your appointment.
What should I bring to my evaluation?
Bring a photo ID, relevant diagnosis and treatment records, and a current medication and supplement list. Have a working email address for your state registry account.
Permanent adult residents use a valid Florida driver license or Florida ID for the state application. Seasonal residents need acceptable alternative residency documents. See OMMU’s documentation checklist.
What are the separate state card fees?
Florida charges $75 for an initial or annual renewal card application, plus a $2.75 online payment convenience fee. These payments go to the state’s application system, not Elite Health DPC. Dispensary products are purchased separately.
How often do I renew my certification and state card?
Physician certification requires reassessment at least every 30 weeks. The renewal fee at Elite Health DPC is $99 for eligible members.
Your state ID card renews separately each year. Applications can begin 45 days before expiration. Paying the clinic fee does not renew your state card.
Can my first appointment be virtual?
The initial certification requires an in-person examination. Florida allows certain subsequent renewal evaluations by telehealth with the physician who previously examined and certified you in person. Ask our team which appointment format is appropriate for you.
What if I already have a Florida medical marijuana card?
Bring your current card and relevant records. We will review your existing physician certification and discuss next steps under our member-only program.
Does membership guarantee certification?
No. Membership and payment do not guarantee approval. Certification depends on medical eligibility and your physician’s assessment of treatment benefits and risks.
When can I start?
Contact us to arrange your evaluation. Purchasing requires state application approval and an active physician order. Processing times vary.
