Contact your physician and PA team by phone or messaging for questions and follow-up. Communication and appointment availability follow clinic policies; this is not an emergency service.

As a valued Elite Health DPC patient, you receive preferred pricing on aesthetic services. Enjoy botox at $10 per unit (regularly $12), $75 off each syringe of dermal filler, and 15% off PRP and Sculptra treatments.
Medical Marijuana Care
Physician-led evaluations and ongoing care for eligible adult Elite Health DPC members. A three-month minimum DPC commitment is required. Certification requires a qualifying condition and physician approval.
