Direct primary care (DPC) uses a monthly membership for routine primary care. At Elite Health DPC in Jacksonville, members receive office and virtual visits, preventive care, and direct clinician communication. We do not bill insurance for membership services. DPC is not health insurance.
Individual membership is $149 per month. Couples living at the same address pay $239 per month. Additional household members aged 18 and older at the same residence may be added for $99 each per month. Annual billing offers a 10% saving. DPC and health insurance pay for different things. A DPC membership may make routine primary care costs more predictable, but it does not cover hospital care, emergency treatment, or specialist bills. Compare the membership fee plus your insurance and other expected costs. Savings depend on your circumstances.
No. You can join Elite Health DPC with or without insurance. We recommend maintaining appropriate health insurance for services outside the membership, including emergencies and hospitalization. If you have insurance, you may be able to use it for outside labs, imaging, and specialists, subject to your plan.
We do not bill commercial insurance or Medicare for DPC membership services. Contact us before joining if you have Medicare so we can explain the applicable enrollment arrangements. Your outside insurance coverage and benefits remain subject to your plan.
Yes. We offer a family membership option for adults living in the same household. Additional household members aged 18 and older can be added to an individual or couples membership for $99 per person per month, in addition to the base membership fee. All members must share the same residence. We do not provide primary care for children under 18.
Membership includes routine office and virtual visits, annual physical examinations, preventive care, management of acute and chronic conditions, and direct clinician communication. Our DPC membership services page explains the benefits. Certain tests, medications, and procedures have separate charges, which we discuss before proceeding.
Not all healthcare expenses are included. Some laboratory tests, diagnostics, medications, and specialized procedures have separate fees. Specialist visits, emergency care, and hospitalization are outside the DPC membership. We help coordinate appropriate referrals and explain available testing options.
Elite Health DPC is led by Yacob Habboush, MD, MPH, a board-certified internal medicine physician, and Noor Daoud, PA-C. Our physician and PA team provides personalized adult primary care with an emphasis on prevention and shared decision-making.
Yes. We offer a complimentary virtual meet and greet. Text 904-881-0238 or use the meet-and-greet button on our Schedule Now page to request a time. The text is a scheduling request; an appointment is confirmed after our team responds.
Existing patients can schedule an appointment by clicking Schedule Now and selecting Existing Patient, or by calling or texting their clinician at the direct number provided to them.

Yes. Elite Health DPC lists a business membership at $109 per member per month with a minimum of four members. Contact us to confirm eligibility and enrollment details. DPC provides primary care access and does not replace an employer health insurance plan.
You may cancel with 30 days’ written notice sent to support@elitehealthdpcjax.com. There is no initial enrollment fee. A re-enrollment fee applies if you return. Ask for the current membership agreement before enrolling.
Revive Aesthetics in Jacksonville offers wrinkle relaxers, dermal fillers, PRP, Sculptra, PDRN, and microneedling. These services are separate from routine DPC membership. Elite Health DPC members receive preferred pricing on eligible wrinkle relaxers and dermal fillers, plus 15% off PRP and Sculptra treatments. Both models may offer more personal access through a membership fee. DPC generally charges directly for included primary care and does not bill insurance for those services. Yes. Medical marijuana care is available exclusively to active Elite Health DPC members aged 18 and older, with a three-month minimum DPC commitment. Initial certification is $175 and certification renewals are $99, separate from membership dues, state card fees and product purchases. Certification requires a qualifying condition and physician approval.
