With this membership, both of you will have full access to Direct Primary Care benefits, including office visits, personalized care, and direct communication with your provider.

Business Membership

For Jacksonville businesses enrolling at least four members. This membership provides access to routine primary care and direct clinician communication. Contact us to confirm eligibility and enrollment details. DPC complements, but does not replace, employer health insurance.
Annual billing offers a 10% saving. Additional household members aged 18 and older who share the same residence may be added to an individual or couples membership for $99 per person per month.
What is included—and what costs extra?
Membership includes routine office and virtual visits, annual physicals, preventive care, and ongoing management of acute and chronic conditions. Review all membership benefits.
Some laboratory tests, diagnostics, medications, and procedures have separate charges. Specialist visits, emergency treatment, and hospital care are not covered by membership. We discuss additional clinic charges before proceeding.
